Background : Based on submission experience, researcher have a lot of patient grievance in central opname which is many patient are complain caused of the long duration of waiting for the inpatient room because the bed are full which is reconciliation hospital. The long waiting duration as long as a night only to get the inpatient room. Objective : To describe how about the process of patient transfer, what obstruction from the process of patient transfer, factor about the obstruction of patient transfer, and what effort from the staff about obstruction of patient transfer. Methods : The type of research is descriptive research with qualitative approach and case study design. There are 6 subjects in this research, the central opname staff, nurse, doctor, room manager and 2 patient. Object of this research is standard operational procedure of patient transfer, process of patient transfer, and the coordination between staff related the patient transfer. Data accumulation resort to interview, observation and documentation study also use the technique triangulation. Results : The process of patient transfer divided from emergency room to inpatient room, from inpatient room to operation room and to the intensive room. Patient transfer among by pekarya (non medic staff) for non emergency patient, and among by paramedic for emergency patient. Obstruction of process patient transfer is the long waiting duration of transition patient around 6-10 hours, in fact 1-3 nights. The factor are overdue of doctor’s visit schedule to inpatient room which is after polyclinic closed its 2 p.m, the presence of reconciliation patient without confirmation, duplication of patient in line, and the take care of discharge patient so long. The certain from the staff are by entrance to waiting room list, reconciled patient, and deposited class. Conclusion : The process of patient transfer is appropriate with standard operational procedure of patient transfer, waiting time of transition patient not appropriate with the regulation which is 6 hours waiting. Doctor’s visit schedule not appropriate with hospital standard minimum service at 8 a.m. ‘till 2 p.m. the certain not effective yet to relieve the waiting time of transition patient.
